Data Privacy Agreement

I understand that etrials, Inc., 4000 Aerial Center Parkway, Morrisville, NC 27560 is the controller of the personal data I provided herewith. Etrials and its affiliates shall process my personal data for the purposes of recruitment and selection. My personal data will be retained as long as necessary for the purposes it has been collected, but not longer than 1 year in countries covered under the EU Directive on the Protection of Personal Data (“EU Directive”). I understand that I can request access to and/or rectification of my personal data by contacting the Manager of Human Resources in writing at the address mentioned above. By accepting this form, I consent that my personal data may, for the purposes of recruitment and selection, also be transferred to other entities of the etrials company, including to entities located in countries that, under the EU principles, do not offer an adequate level of protection of personal data. In addition, I agree that my personal data may later be processed by an etrials third party contractor for the purposes mentioned above. The supply of my personal data to etrials is voluntary. I acknowledge that if I do not wish etrials to collect, process or disclose such data in accordance with this statement, I will not attempt to provide etrials with such data.
